What Google announced

In a September 24, 2026 post on its official blog, signed by Senior Product Manager Shamira Sridharan Weaver, Google said Health Guardian, its new proactive health and safety toolset, is rolling out in the Google Health app so Pixel Watch users can manage their health right from the wrist. Google says that starting the week of the announcement, users can set up blood pressure trends, insulin resistance trends and a sleep breathing quality metric in the app. Insulin resistance means the body responds less well to insulin, the hormone that helps control blood sugar.

Google says these features are backed by clinical research and run continuously in the background, tracking subtle physiological changes over long periods. These claims come from Google's own announcement.

How Breathing Emergency Detection works

Google calls Breathing Emergency Detection a first-of-its-kind feature. It is designed to automatically prompt a call to emergency services when the user's blood oxygen drops severely. According to Google's footnotes, the feature has CE marking in the EU, the mark showing a product meets EU requirements. It has not been cleared or evaluated by the FDA, the US Food and Drug Administration. It is currently offered only in select European markets.

What it means for everyday users

  • Existing Pixel Watch 3, 4 or 5 owners: according to Google, you can set up the new trend features in the Google Health app at no extra cost, with first reports expected in early October.
  • Fitbit Air users: according to Google, blood pressure and insulin resistance trends will be available only later this year and require a Google Health Premium subscription.
  • Trend reports are not real-time: Google notes blood pressure and insulin resistance insights arrive on the 1st of each month and require enough days of wear.
  • Users outside Europe: Breathing Emergency Detection is currently limited to some European markets.
  • Pregnant people: Google notes blood pressure trends, insulin resistance trends and the breathing quality metric should not be used by people who are pregnant.

FAQ

Does Health Guardian cost anything?

According to Google, blood pressure trends, insulin resistance trends and the sleep breathing quality metric are free for Pixel Watch 3, 4 and 5 users. Fitbit Air users need a Google Health Premium subscription to see blood pressure and insulin resistance trends. Google also notes some features may require a separately sold subscription.

When will I get my first trend report?

Google says the first trend reports are expected in early October. After that, blood pressure and insulin resistance insights arrive on the 1st of each month. Insulin resistance needs at least 7 days of data in the month, and blood pressure needs at least 5 consecutive days of data within a week.

Can the watch's blood pressure trends replace a blood pressure monitor?

No. Google notes the feature does not provide real-time clinical readings, is not a pre-screening tool for hypertension, and should not be used to adjust medication or treatment plans. For measurement or diagnosis, consult a medical professional.

Can users outside Europe use Breathing Emergency Detection?

According to Google, Breathing Emergency Detection is currently available only to Pixel Watch 4 and 5 users in some European countries. The announcement does not mention a rollout timeline for other regions.

Have these features been recognized by regulators?

Google addresses regulators only for Breathing Emergency Detection: it has CE marking in the EU but has not been cleared or evaluated by the US FDA. Google says the features are backed by clinical research, but that claim comes from Google itself.
